Rien de spectaculaire, mais une douleur suffisamment pr\u00e9sente pour changer sa fa\u00e7on de bouger. Depuis, il fait attention. Il \u00e9vite certains gestes, se rel\u00e8ve plus lentement, limite ses activit\u00e9s. On lui a dit de se reposer, alors il se repose. Au fond, cela semble logique. Moins bouger devrait laisser le temps au corps de r\u00e9cup\u00e9rer.\u00a0 Les premiers jours, \u00e7a le rassure. Mais tr\u00e8s vite, quelque chose change. Ce n\u2019est pas seulement la douleur qui persiste. C\u2019est aussi une sensation plus diffuse, plus insidieuse. Bouger devient doucement moins naturel et plus co\u00fbteux. Comme si son dos r\u00e9pondait moins bien, comme si chaque mouvement demandait un effort suppl\u00e9mentaire.<\/p>\n\n<p>Ce que Marc ressent n\u2019a rien d\u2019anodin. Pendant longtemps, le repos a \u00e9t\u00e9 recommand\u00e9 en cas de lombalgie. Pourtant, les recommandations actuelles sugg\u00e8rent plut\u00f4t de <strong>maintenir ou de reprendre progressivement les activit\u00e9s<\/strong>, m\u00eame en pr\u00e9sence de douleur [1,2].\u00a0 Lorsque l\u2019on bouge moins, les muscles perdent en endurance, la coordination devient moins efficace, et la tol\u00e9rance \u00e0 l\u2019effort diminue. Peu \u00e0 peu, ce qui \u00e9tait facile devient plus difficile. Ce ph\u00e9nom\u00e8ne, bien d\u00e9crit dans la litt\u00e9rature, participe \u00e0 ce que l\u2019on appelle la <strong>d\u00e9sadaptation fonctionnelle <\/strong>[3]. Les observations scientifiques montrent que la lombalgie s\u2019accompagne souvent d\u2019une diminution de la mobilit\u00e9 et d&#8217;une alt\u00e9ration des capacit\u00e9s musculaires [4,5].\u00a0 Mais l\u2019histoire ne s\u2019arr\u00eate pas l\u00e0. La douleur elle-m\u00eame modifie la mani\u00e8re dont le corps va se mettre en mouvement. La revue de Hodges (2011) ont montr\u00e9 que, face \u00e0 une douleur, le syst\u00e8me moteur adopte <strong>une strat\u00e9gie protectrice <\/strong>[6]. L\u2019activit\u00e9 musculaire se redistribue, le tronc devient plus raide et les mouvements perdent en fluidit\u00e9. A court terme, cette adaptation est utile. Elle vise \u00e0 prot\u00e9ger les structures sensibles. Mais lorsqu\u2019elle persiste, elle rend le mouvement moins efficace, parfois m\u00eame plus contraignant.<\/p>\n\n<p>Marc ne le sait pas, mais son corps est en train de s\u2019adapter. Et ces adaptations, bien qu\u2019intentionnellement protectrices, contribuent \u00e0 rendre le mouvement plus difficile. Plus il \u00e9vite, plus il perd en capacit\u00e9. Plus il perd en capacit\u00e9, plus bouger devient co\u00fbteux. Peu \u00e0 peu, un cercle s\u2019installe, proche de ce que d\u00e9crit le <strong>mod\u00e8le scientifique fear-avoidance<\/strong>, o\u00f9 la douleur est interpr\u00e9t\u00e9e comme une menace et conduit \u00e0 limiter l\u2019activit\u00e9 [7]. Dans ce contexte, dire \u00e0 Marc de \u201crester actif\u201d est scientifiquement juste, mais reste incomplet. Car la vraie difficult\u00e9 n\u2019est pas de comprendre qu\u2019il faut bouger. C\u2019est de <strong>pouvoir<\/strong> le faire sans que chaque mouvement devienne une \u00e9preuve. L\u2019enjeu est alors de recr\u00e9er des conditions dans lesquelles le mouvement redevient accessible. Non pas en supprimant toute douleur, mais en la rendant compatible avec l\u2019activit\u00e9. C\u2019est pr\u00e9cis\u00e9ment dans cette logique que s\u2019inscrivent certaines approches de r\u00e9adaptation modernes, qui cherchent \u00e0 moduler la charge et \u00e0 faciliter une reprise progressive du mouvement.<\/p>\n\n<p>Des solutions comme <strong>l\u2019orth\u00e8se lombaire dynamique TEOS <\/strong>s\u2019inscrivent dans cette perspective. En r\u00e9duisant temporairement la contrainte m\u00e9canique appliqu\u00e9e \u00e0 la colonne, elles permettent de diminuer la sensation de charge et de rendre certains mouvements plus tol\u00e9rables.
